Colonic Polyps

Introduction
  • Classified into several subcategories
    • hamartomas
      • benign
      • found in juvenile polyposis and Peutz-Jeghers syndrome
    • inflammatory polyp
      • benign
      • found in inflammatory bowel disease
    • neoplastic
      • most commonly adenomas
        • tubular adenoma
          • smallest malignant potential
          • usually pedunculated
        • tubulovillous adenoma
          • usually pedunculated
        • villous adenoma
          • greatest malignant potential
          • usually sessile
  • Mean age of patients is 55
  • Sigmoid and rectum most common site.
Presentation
  • Symptoms
    • most are asymptomatic
    • can present with rectal bleeding
Evaluation
  • Sigmoidoscopy / colonoscopy
Differential
  • Polyposis syndromes
    • FAP
    • Peutz-Jeghers
    • Gardner's syndrome consists of polyposis with multiple soft tissue tumors:
      • desmoid tumors
      • osteomas of mandible or skull
      • sebaceous cysts
    • Cronkhite-Canada syndrome consists of polyposis with:
      • alopecia
      • fingernail dystrophy
      • cutaneous hyperpigmentation
    • Cowden syndrome consists of polyposis with:
      • breast cancer
      • thyroid cancer
    • Turcot's syndrome consists of polyposis with:
      • medulloblastoma or glioma
Treatment
  • Colonoscopic polypectomy or laparotomy with resection.
  • If adenocarcinoma found in polyp only, then colectomy not required.
