A 45-year old woman is referred to a neurologist for 6 months of muscle weakness. The patient describes a progressive, symmetrical onset weakness that is mainly affecting her thigh muscles. She reports difficulty rising from bed in the morning and climbing stairs. Additionally, the patient also reports increased constipation and dry mouth. On physical examination, neither ptosis nor diplopia are noted. Deep tendon reflexes (DTR) are absent when tested but return after a few minutes of exercise.
