A 47-year-old male competitive cyclist presents to his primary care physician complaining of 2 months of decreased grip strength in his left hand. He states that he has associated pain and numbness on the medial aspect of his hand, which has been getting progressively worse. He has never had similar symptoms before. He has no chronic medical conditions and takes no medications. On physical exam, he has 5/5 strength bilaterally in elbow and wrist flexion/extension, but when asked to extend the fingers of his left hand he has the findings as pictured in Figure A.
