This patient's clinical presentation is consistent with lyme disease. tRNA is covalently bound to amino acids at the 3' CCA tail.

Translation from mRNA to proteins is accomplished using tRNA molecules, which act as carriers of individual amino acids. A "loaded" tRNA molecule is covalently linked to an amino acid at the 3' CCA tail, and contains a specific anticodon sequence between the D and T loops. This anticodon sequence allows the tRNA to be targeted to the correct location, such that specific mRNA sequences produce specific amino acid additions.

Colussi et al. published a crystal structure of an unusual tRNA-like structure that is found in the turnip yellow mosaic virus. They note that the structure has two structural areas: one that is similar to a typical tRNA, and functions as expected as an aminoacyl transferase, and a second conformation that allows the tRNA to act as a promoter.
